| Medical / Membership Form | Please keep the club informed of any medical changes to ensure your child is physically fit to participate, particularly after illness or injury. This includes Covid19 symptoms. Gymnastic activities have an inherent risk of injury and although the club will endeavour to minimise any risk, accidents may still happen. This form helps you consent to your child having emergency first aid administered or medical assistance if required. | Download |
